ALREADY HAVE A NEW PATIENT APPOINTMENT?
We are so happy to welcome you to our Platz family! Once you have made an appointment for your pet,
you will need to fill out some paperwork for us. This will be put in your pet's chart.
To assist in a smooth and successful visit, please have the form filled out prior to your arrival.
Any previous records are appreciated at the first visit. You can bring a copy, fax them or send them in an email.
They will be put in their chart so our team can provide the best care possible.
